Immunization

IMMUNIZATION POLICY

For school enrollment, a parent or guardian shall provide one of the following two items:

1.     Current, up-to-date immunization records.  
   
2.     A guide to required immunizations may be found at this link:   Oklahoma Health Services
3.     A completed and signed exemption form.
